CUSTOMER CONSENT FORM

Dear Sir/Mdm,

We at Income value our customers and would love to share exclusive offers (such as rewards, privileges, events, and discounts) and information about products and services (“Marketing and Promotional messages”) offered by Income, its affiliates* and business partners and NTUC Enterprise group of social enterprises (“NE Group”) that may be useful to you and your family.

By providing your particulars in this form, you consent and agree to the collection, use and disclosure of your personal data by Income, its representatives, agents (collectively “Income Persons”), affiliates, appointed service providers and business partners and NE Group for the purposes of (i) sending you Marketing and Promotional messages; (ii) contacting you to conduct survey; and (iii) Income Persons offering you financial planning services via Email, Phone call and Phone messages**.

*“affiliate” means, with respect to any person, any other person directly or indirectly controlling, controlled by, or under common control with, such person. The word “control” (including its correlative meanings, “controlled by”, “controlling” and “under common control with”) shall mean, with respect to a corporation, the right to exercise, directly or indirectly, more than 50 per cent. of the voting rights attributable to the shares of the controlled corporation and, with respect to any person other than a corporation, the possession, directly or indirectly, of the power to direct or cause the direction of the management or policies of such person. *Phone messages include text, picture, video, and audio message that are sent to your telephone number via SMS, MMS, or messaging apps such as WhatsApp, Telegram or WeChat.

We will treat the consent given in this form by you to be valid for 60 days only. After which, we will seek further consent from you to follow up where necessary.

Your consent to receive Marketing and Promotional messages above is in addition to, and does not vary or supersede any previous marketing consent which you may have provided to Income.

Your Particulars

You may withdraw your consent at any time by submitting your request at agency@income.com.sg. Income will process your request within 10 days, and you will stop receiving Marketing and Promotional messages after 21 days only for the mode(s) of communications indicated in your request. You may refer to Income’s Privacy Policy for more information, including access and correction to personal data and consent withdrawal.
