What counts as a pre-existing condition under maid insurance

A pre-existing condition is generally any medical condition that existed before the policy start date, whether or not it was formally diagnosed at the time. However, the exact definition can vary between policies, so it is worth reading the fine print before you commit to a plan.

Based on Income's Dometic Helper's policy conditions, pre-existing medical condition means any injury or sickness, including any complications which may arise: 

a) which you or your domestic helper knew or should reasonably know about, including symptoms which existed, before the start of the policy or before you purchased the top-up of the hospital and surgical expenses benefit limit, whichever is applicable; 

b) which your domestic helper has received diagnosis, consultation, medical treatment or prescribed drugs for within the 12 months, before the start of the policy or before you purchased the top-up of the hospital and surgical expenses benefit limit, whichever is applicable; or 

c) for which your domestic helper has been asked to get medical treatment or medical advice by a medical practitioner within 12 months, before the start of the policy or before you purchased the top-up of the hospital and surgical expenses benefit limit, whichever is applicable.

How maid insurance with pre-existing condition coverage works

In Singapore, most maid insurance policies won't cover a helper's pre-existing conditions right away. To get coverage, you usually have to meet one or both of these conditions:

  • 12-month waiting period: The policy must be active for at least one full year before any pre-existing conditions are covered.

  • Continuous insurer: If you switch to a new insurance company when renewing her contract, the 12-month clock resets. To keep her conditions covered continuously, you must renew with the same insurer.

This means hospital and surgical expenses (including day surgery) arising from pre-existing conditions becomes eligible for coverage under the policy, up to the hospital and surgical benefit limit applicable, at the time the pre-existing medical condition first occurred or at the time she incurs the hospital and surgical expenses; whichever is lower but in no event below the basic limit.

Pre-existing condition coverage comes with nuances that are easy to miss, especially for first-time employers. Keep these four key factors in mind so you are not caught off-guard.

1. Waiting period

Usually, standard domestic helper policies exclude pre-existing medical conditions when a helper first starts working. However, this exclusion is lifted once the helper has been employed as a foreign domestic helper in Singapore for more than 12 consecutive months. 

This means that if your helper is in her first year of work in Singapore and requires hospitalisation or surgery for a condition she had before arriving, you will have to foot the entire medical bill out of pocket. Once she crosses the 12-month mark, the policy will begin covering those pre-existing conditions under the Hospitalisation and Surgical Expenses benefit.

2. Reset of waiting period 

Your waiting period may reset with a new insurer if you switch insurers at renewal. This means helper who has completed 12 months with one insurer may not automatically carry that medical history to a new provider so do check whether the new policy treats as a new entrant. 

To ensure continuous, uninterrupted coverage for an ongoing or chronic condition, you may consider staying with the same insurer upon renewal.

3. Coverage for pre-existing conditions

It is a common misconception that once the waiting period ends, all medical costs for that pre-existing condition are covered. However, the pre-existing condition benefit applies to inpatient hospital and surgical expenses only.

Even after the qualifying period, outpatient treatment and daily maintenance medications (like blood pressure or diabetes drugs) for pre-existing conditions are generally not covered. You will need to pay for these routine clinic visits for your helper.

4. Insurance renewal terms

When it is time to renew your helper's policy, you cannot assume that the terms from the previous year will apply identically to the next. Carefully check your renewal documentation for any new co-payments, revised claim limits, or updated exclusions regarding her specific ongoing conditions before you pay the premium.
