By completing and submitting this online form1 to register for “Rider Changes Implemented in April 2026: What You Should Know?” (the “Event”) organised by Income Insurance Limited (“Income Insurance”) and NTUC, you consent and agree to:
1. the collection, use and disclosure of your personal data by:
(i) Income Insurance, our representatives and third-party service providers (referred to in Income Insurance’s privacy policy at income.com.sg/privacy-policy) for the purposes of administering or organizing the registration, logistic planning, and on any follow-up action or identity verification matters pertaining to the Event, which includes contacting you (via email, phone call and phone messages*);
(ii) Income Insurance, its representatives, and agents for the purposes of marketing which consists of news releases, website updates, and social media content on Income Insurance’s, its representatives’, and/or agents’ channels including but not limited to Facebook, Instagram and TikTok.
(iii) Income Insurance, its representatives, and agents (collectively “Income Insurance Persons”), appointed service providers, business partners and NE Group for the purposes of: (a) sending you marketing and promotional messages; (b) contacting you to conduct survey(s); and (c) Income Insurance Persons offering you financial planning services, via postal mail, email, phone call and Phone messages*.
(*Phone messages include text, picture, video and audio message that are sent to your telephone number via SMS, MMS or messaging apps such as WhatsApp, Telegram or WeChat.)
2. Income Insurance disclosing your personal data to NTUC for their collection, use and disclosure of such personal data for the purposes of verifying your NTUC Union membership, and where applicable, administering the delivery of any gift item to you.
Your consent to receive marketing messages above is in addition to, and does not vary or supersede any previous marketing consent which you may have provided to Income Insurance. All consent given to Income Insurance in receiving Marketing and Promotional messages will remain valid until Income Insurance is notified that the consent has been withdrawn.
Where the personal data of another person(s) (for example, personal data of your family member) is provided by you (whether in this or subsequent submissions) to Income Insurance Persons, you represent and warrant that you have obtained their consent for the collection, use and disclosure of their personal data; and you are authorised to give any authorisation or approval on their behalf, for the purposes as set out in this Personal Data Use Statement.

In relation to your personal data collected by Income Insurance, you may submit a request at any time at income.com.sg/enquiry for the correction of, access to and withdrawal of consent relating to such personal data. Income Insurance will process your request for consent withdrawal within 10 days, and you will stop receiving Marketing messages after 21 days only for the mode(s) of communications indicated in your request. Please refer to Income Insurance’s privacy policy available at income.com.sg/privacy-policy for more information. You agree and understand that Income Insurance’s privacy policy available on its website may be amended, supplemented and/or substituted by Income Insurance from time to time.
2. Note: In relation to your personal data collected by NTUC, the use and disclosure of your personal data by NTUC will be subject to NTUC’s privacy policy. Please submit your request separately to [email protected] and refer to their privacy policy for more information. NTUC will process your request accordingly.
