Please Note: These forms are to be used in conjunction with the new Nomination of Beneficiaries under the Insurance Act only. Please read the instructions carefully before completing any Form.

• Form 1 : Trust Nomination
  Note:
  Making a trust nomination is a serious undertaking. The policyholder loses all rights and control over the policy. He cannot give instruction on the policy or receive any payouts from any surrender, maturity, loan, or claim event. And he cannot change his nomination without consent from trustee or nominees. Please call 6788 1122 for advice.
  
  
• Form 2 : Revocation of Trust Nomination
  Note:  Please use this form to revoke a Trust Nomination only which was made using Form 1.
  
  
• Form 3 : Appointment or Revocation of Trustee
  Note: Please use this form to:
  
  
  • appoint 1 or more trustees in addition to any trustee appointed earlier under your Trust Nomination (made using Form1);
    
    
  • only revoke any trustee(s) appointed earlier under your Trust Nomination (made using Form 1) provided there is at least one remaining trustee;
    
    
  • revoke any trustee(s) appointed earlier under your Trust Nomination (made using Form 1) and appointing 1 or more trustees to replace the trustee whose appointment is being revoked.
    
    
• Form 4 : Revocable Nomination
  Note:  Please use this form to make a Revocable Nomination.
  
  
  • If there is any existing Revocable Nomination or section 45 nominations under the Co-operative Societies Act, making a subsequent fresh Revocable Nomination has the effect of overriding the existing nomination.
    
    
• Form 5 : Revocation of Revocable Nomination
  Note: Please use this form to revoke a Revocable Nomination (made using Form 4) only but do not wish to make a new Revocable Nomination.
  
  
• Form 6 : Notice of Revocation of Revocable Nomination
  Note: Please use this Form to give Notice of Revocation of Revocable Nomination if the policy owner has:
  
  
  1. assigned, encumbered or otherwise dealt with the policy or any interest under the policy; or
     
     
  2. after making a Revocable Nomination, made a will in accordance with the Wills Act,
     
     as an existing Revocable Nomination would be deemed as revoked.
     
     
• Deed of Revocation (Section 73 CLPA)
  Note: Please use this form to revoke a nomination made under section 73 of the Conveyancing and Law of Property Act, Singapore (CLPA).
  
  
• Revocation of Nomination (Section 45 CSA)
  Note: Please use this form to revoke a nomination made under section 45 of the Cooperative Societies Act (CSA) and only if you do not wish to make a new nomination.
