STUDENT PROTECTION PLAN/ STUDENT SPORTS INJURY PLAN/ JUNIOR PROTECTION PLAN CLAIM

Documents required:

1. Death Claim

• Student Protection Plan/Student Sports Injury Plan / Junior Protection Plan Claim Form with school's/centre's stamp (to be completed by next-of-kin)
• Certified True Copy of Death Certificate (for overseas death, the original Death Certificate must be certified by your lawyer or any Notary Public)
• For Singaporeans who have died overseas, the death certificate is to be certified by and translated into English by the Singapore Embassy in the country of death.
• Letter from Immigration and Checkpoint Authority (ICA) - this letter is issued by ICA for Singaporeans or Permanent Residents (PR) who died overseas. It confirms receipt of the Singapore IC, Passport and overseas Death Certificate.
• Repatriation Report (if body was repatriated to Singapore for cremation/burial)
• NRIC(s)/ BC(s)/ Passport(s) of Claimant(s)
• Proof of Claimant's relationship with Deceased

2. Permanent/Partial Disability Claim

• Student Protection Plan/Student Sports Injury Plan / Junior Protection Plan Claim Form with school's/centre's stamp (to be completed by claimant)
• Medical reports/ Laboratory reports
• Hospital Discharge Summary
• Medical Certificates
• Medical reports
• Newspaper Clipping and Police/Accident Report (if disability was due to accidental or violent causes)

3. Medical Expenses Claim

• Student Protection Plan/ Student Sports Injury Plan / Junior Protection Plan Claim Form with school's/centre's stamp
• Hospital discharge summary
• Medical Certificates
• Original final hospital bills & receipts
• Medical reports, accident reports if available
• A copy of the reimbursement letter/discharge voucher from the insurer/employer (If there is previous reimbursement from another insurer/employer)
