Making A Claim

Life & Health Insurance Claim

  1. Death Claim

    Documents required:

    1. Death Claim Form (to be completed by next-of-kin)
    2. Death Certificate
    3. Burial Certificate, and ICA (Immigration and Checkpoints Authority) Acknowledgement Letter on update of death status (where death had occurred overseas)
    4. Proof of Claimant's relationship with Deceased (spouse, child, parent, sibling) such as copy of Marriage/Birth certificates and Identity Cards
    5. Newspaper Cutting and Police Report (if death was due to accidental or violent causes)
    6. Copy of Will, if applicable

  2. Permanent & Total Disability Claim / Permanent Incapacity (PI) Claim (DPS Policy)

    Documents required:

    1. Medical/Accident/Living/Permanent & Total Disability Claim Form
    2. Attending Physician's statement (APS)
    3. Medical reports/ Laboratory reports/ Hospital Discharge Summary
    4. Termination letter from last employer OR CPF Statement showing last employment contribution (for DPS policy only)
    5. CPF Contribution Statement for the past 15 months (for DPS policy only)

      Please note that (a) & (b) must be completed by the claimant.

  1. Dread Disease (Living) Claim

    Documents required:

    1. Medical/Accident/Living/Permanent & Total Disability Claim Form
    2. Attending Physician's Statement (APS)
    3. Medical reports/ Lab reports/ Hospital Discharge Summary if any

      Do note that (a) & (b) must be completed by the claimant.
      Please also use the specific APS form if claimant is claiming under the following medical conditions: Cancer, Heart Attack, Kidney Failure, Stroke, Coronary artery By-pass Surgery.

  2. Student Protection Claim

    Documents required:

    1. Student Protection Plan Claim Form
    2. Police report where relevant
    3. Death claim:
      - death certificate, autopsy report, coroner's findings, birth certificate
    4. Permanent disability claim:
      - medical report
    5. Medical expenses claim:
      - Original medical bills & receipts
    6. If these have been given to another insurer/employer, please provide:
      - photocopy of the bills
      - reimbursement letter/discharge voucher from the
      - insurer/employer indicating the amount paid by them

  3. Surrenders of Policies

    For surrenders done at Branches or Main Office, please bring:

    1. NRIC/Passport for identification
    2. Name of Bank/Branch/Bank account number for GIRO credit

      You will be required to sign on a form indicating that you have understood the alternative options and disadvantages of surrendering your policy. This form helps you to make a more informed decision before terminating your policy.

      For traditional policies, the form is: What you should know about Early Surrender of your Insurance Policy
      For unit linked policies, the form is: Early Full/Partial Withdrawal of your ILPs.

      If you are surrendering a traditional life policy, you will also be required to sign on a Surrender Discharge Voucher indicating the cash value of your policy.

  4. Maturity Claims

    You will be informed (by mail) of the estimated gross maturity value about 3 weeks before the maturity date. The maturity value shall be paid on or before the maturity date.

  5. Instalment Payments (Anticipation Policy)

    You will be informed about 3 weeks before the instalment due date and given an option to take out the instalment amount or to deposit it with Income at the prevailing interest rate. The instalment shall be paid on or before the instalment due date.

  6. Medical/Personal Accident Claim

    This claim may be made for the following plans:

    1. Incomeshield (Integrated *)
    2. Incomeshield (non-Integrated *)
    3. Family Plus
    4. Annuity Hospital & Surgical
    5. Managed Healthcare System
    6. i-Medicare
    7. Group Hospital & Surgical
    8. Hospital Benefit (rider)
    9. Accident Benefit (rider)
    10. * Integrated Incomeshield refers to a Medisave approved medical plan which premiums are paid for using Medisave (fully or partially) and non-integrated Incomeshield refers to policies which premiums are paid using cash.

      Claims Procedure for (a)

      • At the point of admission, inform the admission staff at the hospital/clinic of coverage under Incomeshield.
      • The hospital staff should provide a PMI claim Form to the policyholder/insured to complete and sign.
      • On discharge, the hospital will electronically file the claim through a centralized system, known as the MediClaim System.
      • Claims information and details of the bills will be transmitted to NTUC Income via the MediClaim System.

      Documents required for b to i:

      • Medical/Accident/Living/Permanent & Total Disability Claim Form
      • Hospital discharge summary/Medical reports (if any)
      • For claims under b, c, d, e, f and g, please submit a copy of the final hospital/medical bills.
      • If there is previous reimbursement from another insurer/employer, please provide a copy of the reimbursement letter/discharge voucher from the insurer/employer
      • For claims under h & i, please submit the medical certificates

      Terms and Conditions for Letter of Guarantee (LOG)

      a) Incomeshield
      b) iMedicare

      Please be informed that the Letter of Guarantee is intended to assist the Insured in the admission process. It waives the need for partial or full deposit required by the hospital for hospitalisation or surgery.

      Please note that LOG is not a policy benefit and does not form part of the policy contract. The issuance of LOG is not an admission of claim liability or the amount of claim liability.

      NTUC Income reserves the right to deny issuance of LOG.

      Hospital reserves the right not to accept our LOG and to collect additional payment as required by their internal practices even if a LOG is provided.

  7. Copay Assist Claim

    Documents required:

    1. Copay Assist Claim Form
    2. Photocopy of final hospital/medical bills
    3. Reimbursement letter/discharge voucher from the insurer/employer indicating the amount paid by them.
