1. Liability Claim
   

   This claim may be made under the following policies:
   

   1. Public Liability
   2. Bailee's Liabilty
   3. Error & Omission etc

   If the estimated claim amount is more than $5000 or the incident is serious, please call us at 6788-6616. We may require a survey to be carried out.

   Documents required:
   

   • Property/Liability Claim Form
   • Letter of claim from the third party with supporting documents if any (eg. invoices of items, quotations for repair)
   • Photographs of damage

2. Medical/Personal Accident Claim
   

   This claim may be made under the following policies:
   

   1. Overseas Student
   2. Foreign Maid
   3. Golf

   Documents required: (whether you are covered for the listed benefits depends on policy you have taken)
   

   • Medical/Personal Accident Claim Form
   • Police report where applicable
   • Death claim:
     • death certificate, autopsy report, coroner's findings.
     • Please provide proof of relationship between the insured and the claimant eg. copy of marriage certificate/birth certificate
   • Permanent disability claim:
     • medical report
   • Temporary disability claim:
     • medical leave certificates
   • Medical Expenses claim:
     • original medical bills & receipts
     • If these have been given to another insurer/employer, please provide:
       • photocopy of the bills
       • reimbursement letter/discharge voucher from the insurer/employer indicating the amount paid by them
   • Compassionate Visit Expenses/Study Interruptions claim (under Overseas Student Plan):
     • copy of hospital bill/death certificate, documents showing insured's relationship with the deceased/hospitalised person, copy of invoice for purchase of airticket, copy of airticket, documents showing tuition fees paid
   • Loss of Luggage/Personal Effects (under Overseas Student Plan):
     • report made to airline/airport, receipts of items claimed
   • Liability (under Overseas Student Plan):
     • third party's written claim if received, photographs of damage caused, invoices/receipts
   • Repatriation/Termination Expenses (under Foreign Maid Package):
     • death certificate/doctor's confirmation of permanent disablement or prolonged terminal or serious illness preventing the maid from discharging her duties, invoices of expenses claimed
   • Hospital & Surgical Expenses (under Foreign Maid Package):
     • original in-patient bills & receipts

3. Pet Insurance Claim
   

   Documents required:

   • Medical/Personal Accident Claim Form
   • Accidental Death claim:
     - death certificate
   • Veterinary Fees:
     - original bills
   • Cremation/Burial Expenses:
     - documents showing death by accident, invoice/receipt for cost incurred
   • Boarding Expenses:
     - copy of insured's hospital bill, document showing cause of hospitalisation, invoices/receipt of boarding expenses
   • Recovery Cost:
     - invoice/receipt of expenses incurred, copy of advertisement
   • Third Party Liability:
     - third party's written claim if received, photographs of damage caused, invoices/receipts

4. Property Claim
   
   This claim may be made under the following policies:
   Fire, Theft, Money, Plate Glass, Home, Golfer's, Marine etc.
   
   If the estimated claim amount is more than $5000 or the incident is serious, please call us at 6788-6616. We may require a survey to be carried out.

Documents required:

• Property/Liability Claim Form
• Police report where applicable
• Photographs of damage
• Quotations, Invoices/Receipts of items claimed

5. Travel Claim

   Documents required:

   • Travel Claim Form
   • If you are covered under an annual policy, please provide copies of travel documents showing the commencement and end of the trip eg. airticket
   • Personal Accident claim
     - Police report
     - Death: death certificate, autopsy report, coroner's findings
     Please provide proof of relationship between the insured and the claimant eg. copy of marriage certificate/birth certificate
     - Permanent disability: medical report
     
   • Medical Expenses claim
     - Original medical bills & receipts, police report where applicable
   • Trip Cancellation/Curtailment claim
     - Airtickets and tour booking invoices, receipts, itinerary, letter from travel agent confirming amount of refund
     - Death certificate/medical report of insured or relatives, documents showing insured's relationship with the deceased/injured/sick person or
     - Documents confirming travel agent's bankruptcy/insolvency or
     - Documents confirming hijacking of insured's public conveyance with at least 12 hours' interruption of the journey
     
   • Overbooked Flight/Missed Connection/Travel Delay claim
     - Airticket and airline's confirmation
   • Baggage Delay claim
     - Airticket, report made to the airline/airport, confirmation of duration
   • Personal Baggage claim
     - Police report, receipts of items claimed

6. Work Injury Compensation Claim

   Documents required:

   • Work Injury Compensation Claim Form
   • Police report where relevant, report(s) lodged with Ministry of Manpower
   • Original medical bills & receipts
   • Medical leave certificates
   • Work permit (foreign workers)
   • Contract/invoice for the project/works if you are a sub-contractor
