How to submit a Work Injury Compensation claim

Step 1: Download and complete the Work Injury Compensation Claim Form

Step 2: You can submit the completed claim form with the required supporting documents by the following methods:

• By mail to NTUC Income, Bras Basah PO Box 0132, Singapore 911802
• At any of our branches
• By email tO pcc@income.com.sg (For medical claims, please send the original medical bills to NTUC Income, Bras Basah PO Box 0132, Singapore 911802 or submit at our branches)

Supporting Documents Required

• Police report where relevant, report(s) lodged with Ministry of Manpower
  
• Original medical bills & receipts
• Medical leave certificates
  
• Work permit (foreign workers)
  
• Contract/invoice for the project/works if you are a sub-contractor
