Making A Claim
  1. Death Claim

    Documents required

    1. Death Claim Form with company's stamp (to be completed by next-of-kin)

    2. Certified True Copy of Death Certificate (for overseas death, the original Death Certificate must be certified by your lawyer or any Notary Public)

    3. For Singaporeans who have died overseas, the death certificate is to be certified by and translated into English by the Singapore Embassy in the country of death.

    4. Letter from Immigration and Checkpoint Authority (ICA) - this letter is issued by ICA for Singaporeans or Permanent Residents (PR) who died overseas.  It confirms receipt of the Singapore IC, Passport and overseas Death Certificate.

    5. Repatriation Report (if body was repatriated to Singapore for cremation/burial)

    6. NRIC(s)/ BC(s)/ Passport(s) of Nominee(s)/Claimant(s)

    7. Proof of Nominee's/Claimant's relationship with Deceased

      Nominee/Claimant Documents Required
      Spouse Marriage Certificate
      Parent Birth Certificate of Deceased
      Child Birth Certificate of Nominee/Claimant
      Sibling Birth Certificates of Deceased and Nominee/Claimant

    8. Original Nomination/Beneficiary Form

    9. Newspaper Clipping and Police Report (if death was due to accidental or violent causes)

    10. Last Will of Deceased (if Deceased had left a Last Will)

    For Death claim proceeds made in the name of the nominee(s), a Discharge Voucher will accompany the payment for completion by the Guardian and return to NTUC Income if the nominee(s) is a minor (under 21 years old).

  2. Total & Permanent Disability Claim

    Documents required

    1. Medical/Accident/Living/Total & Permanent Disability Claim Form with company's stamp (to be completed by claimant)

    2. Attending Physician's Statement (APS) (to be completed by attending physician & submitted to us)

    3. Medical reports/ Laboratory reports/ Hospital Discharge Summary

    4. Medically boarded out letter (where applicable)

    5. Newspaper Cutting and Police/Accident Report (if total & permanent disability was due to accidental or violent causes)

    For Total & Permanent Disability claim from all causes, full sum assured plus additional 1% of sum assured for 24 months, till age of 65 years or death whichever is earlier (except self-inflicted injuries within 12 months from policy commencement or reinstatement date) is payable. The claim proceeds are made in the name of the surviving insured member.

  3. Critical Illness (Living) Claim

    Documents required

    1. Medical/Accident/Living/Total & Permanent Disability Claim Form with company's stamp (to be completed by claimant)

    2. Attending Physician's Statement (APS) (to be completed by attending physician & submitted to us)

    3. Medical reports/Laboratory reports/Hospital Discharge Summary

      Please also use the specific APS form if claimant is claiming under the following medical conditions: Cancer, Heart Attack, Kidney Failure, Stroke, Coronary Artery By-pass Surgery, Angioplasty and Other Invasive Treatment for Coronary Artery
